What are pharmacy benefit managers or PBMs?

PBMs serve as the “middlemen” between drug manufacturers and insurance companies. These companies, which are often owned by insurers, claim to negotiate drug prices in order to lower healthcare costs. In reality, PBMs serve as the gatekeepers to insurance companies’ formularies (list of drugs the insurer will cover) and require drug manufacturers to offer rebates in exchange for having their drugs listed. These rebates perversely incentivize PBMs to select drugs based on their rebates rather than patient needs. This system keeps drug prices high for patients, while insurers and their PBMs pocket rebates and profit.
