This was super nostalgic to sub, it's such a good song. "Chizuru" means "a thousand cranes". It's an ancient legend that if someone could fold one thousand origami cranes, they would be granted a wish.

"I want to see it and hear it from your mouth" - Literally "[...] and I want you to let me hear it from your mouth".
"One day I want to forget even the heavy breathing" - "Heavy breathing" is a slightly loose translation. The original describes it as "not being able to breathe well".
"The shadow I saw through the dim space was [...]" - "Space" as in, a space between two things that you can see through (e.g. the space between your fingers, the spaces between grass).
"When my eyes that are reflected on you will lose sight of you - When that day comes..." - I decided to arrange this in a way that matches the original. Because the grammar is different in Japanese and English, it didn't turn out as natural as I would have preferred. So just letting you know that if subtitle timing wasn't an issue, this bit would have been "Even if the day will come when my eyes that are reflected on you will lose sight of you...".
"Seared into my eyes" - A grammatical device that expresses gratefulness (for having it seared into their eyes) is used here (くれる).
"The blurry white is shaking" - Ruki left out the line that follows this. It would be "I feel like I'll even forget how to speak".
"A reality in cold colors" - The kanji say "reality", which is why I stuck to this translation, but Ruki reads them as "now"/"the present".
"I get closer to your very small wish to a thousand cranes" - Like mentioned in the title translation, legend says that if you fold a thousand origami cranes, you will be granted a wish. This is most likely what this line refers to. Also technically, "cranes" aren't specified in this line (just that it's birds), but it appears to be obvious from the context of the song.
"The two that can't become one" - "The two" (which Ruki repeats twice) specifically refers to two people.
